Healthy Changes: A Diabetes Wellness Program is designed for adults aged 50+ with Prediabetes, Type 1, or Type 2 diabetes. Both an educational and support program, the program was developed to assist adults in the day-to-day self-management of diabetes by focusing on diet and physical activity issues.

The program offers weekly group meetings at which participants learn about exercise and physical activity as they relate to diabetes, have an opportunity to discuss their personal goals and achievement of those goals, receive problem solving help and support from other group attendees, and learn about community resources available to them. This is an ongoing program, participants may join at any time.

Sponsored by CSSC, Inc., Healthy Changes is a model health program endorsed by the National Council on Aging’s Center for Healthy Aging. Presented by Colonie Senior Service Centers and the AMC Division of Endocrinology’s Kathryn Godley, MS, RN, CDE, Diabetes Nurse Clinician and Teri Hutson-Mulligan, RD, CDE, Registered Dietitian & Educator.

Healthy Changes: A Diabetes Wellness Program meets on Tuesdays at the Beltrone Living Center, 6 Winners Circle, Albany, New York 12205 from 10:00 am – 11:30 am; There is a suggested contribution of $30 for 26 weeks. For registration materials or additional information call 459-2857, ext. 303
